About the Program
Harmony Centre for Community Living is an approved Passport Transfer Payment Agency (TPA), supporting adults with developmental disabilities to manage their Passport funding and access meaningful supports in their community.
Passport is a Ministry of Children, Community and Social Services (MCCSS) funded program that helps eligible adults participate in everyday life, build skills, and pursue personal goals. Passport funding is individualized and based on assessed need. Eligible individuals must apply through Developmental Services Ontario (DSO).
As a Passport TPA, Harmony Centre helps individuals and families understand their Passport funding agreement, submitexpense claims and invoices, receive reimbursements in a timely and accurate manner, navigate Passport guidelines and reporting requirements, and access information and support to use funding effectively.
In addition to administering Passport funding, Harmony Centre offers fee-for-service supports that can be purchasedusing Passport funding, subject to eligibility and available funding. Fee-for-service options may include one-to-one or small group community participation supports, skill development and life skills coaching, support with employment readiness or community inclusion goals, and structured programs or activities facilitated by trained staff.
We believe Passport funding works best when individuals are supported to make informed choices about how their funding is used. Our Passport services are guided by principles of choice and self-direction, transparency and accountability, respect, dignity, and inclusion, and responsible stewardship of public funds.
We are accountable to individuals and families, as well as to MCCSS, to ensure Passport funding is administered fairly and appropriately.
